FOIA: Freedom of Information Act/Records DirectoryAll FOIA Requests should be directed to Mrs. Jaime Koziol or Mrs. Kerry Gurklies.
Any person requesting records of Lockport Elementary School District 91 may make such a request in person, orally or in writing, at the District Office in Kelvin Grove School, 808 Adams Street, Lockport Illinois 60441. Such a request should be made (to the attention of) Mrs. Jaime Koziol, Superintendent of Schools and designated Freedom of Information Officer at such address. If she is not available, such a request may be made to Mrs.Kerry Gurklies, Executive Secretary to the Superintendent and Secretary to the Board of Education at such address.
Alternatively, any person may mail or use the email contact form below to send a written request to Mrs. Jaime Koziol specifying in particular the records requested to be disclosed and copied. All written requests should be addressed to the Board of Education office at the above address. If you desire that any records are certified you must indicate that in your request and specify which records must be certified. The fees for copies are as follows:
District Office hours of business are Monday through Friday 8:00 AM – 4:00 PM (summer office hours Monday through Thursday 8:00 AM - 4:00 PM). The District Office is closed for business on all official legal holidays observed by Illinois public schools, winter break, and spring break (see District calendar at www.d91.net). |
